Instructions to complete in Acrobat Reader®:  Open this file in Adobe Acrobat Reader®. Select the "hand" tool and move over the form until the hand changes into an I-bar or pointing finger. Type in your information, moving through the form using your TAB key or the cursor. Print the form (making a copy for yourself), attach a current curriculum vitae, and submit the application to your dean. If you do not have the reader, you may download it free from Adobe's Web site.

Instructions to complete in Microsoft Word®:  Download this file and open it in Microsoft Word®. Under the "File" menu choose "Save As," rename the document and save it to your computer. Place your cursor in an entry field and begin typing, using your TAB key or cursor to move between fields to fill in and complete the form. Print the form, attach a current curriculum vitae, and submit the application to your dean. (Instructions for downloading Word files.)
